Email or call Kathy, give her your insurance information, and she will do a benefits check which will let you know the exact nature of your coverage. You will need to mail, scan and email or fax her a signed New Client Registration Form. She needs that in order to confirm your benefits due to HIPPA regulations. For clients who choose to use their insurance, we will submit your claim for you to the insurance company once a month.
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
